Certified Nursing Assistant (CNA) - Home Health & Hospice The CNA provides compassionate personal care and supportive services to patients in their homes, assisted living facilities, long‑term care, and hospital settings under the direction of a Registered Nurse. This role supports patients across the lifespan, promotes comfort and dignity, and helps maintain a safe environment while contributing to an interdisciplinary care team. Key Responsibilities Personal care : Assist with bathing, grooming, dressing, toileting, feeding, mobility, and range‑of‑motion exercises. Safety & environment : Recognize unsafe conditions and report concerns to the supervising nurse before starting or continuing services. Vital signs : Obtain and document blood pressure, temperature, pulse, and respirations. Non‑skilled treatments : Provide routine skin care, non‑sterile dressing changes, and established colostomy care per policy. IADL support : Perform light housekeeping, laundry, meal prep, grocery shopping, and essential errands as outlined in the Plan of Care. End‑of‑life care : Provide comfort, emotional support, and respite for caregivers; assist families through hospice and bereavement processes. Communication & reporting : Observe and report changes in condition; maintain accurate, timely documentation. Team collaboration : Work effectively with nurses, therapists, social workers, chaplains, and other team members. Professional standards : Follow infection control practices, maintain confidentiality, uphold agency policies, and participate in required training. Qualifications High school diploma or GED Current Wyoming CNA certification and listed in good standing on the Nurse Aide Registry Current CPR certification Valid driver's license with a clean driving record Home Health or Hospice experience preferred Skills & Attributes Compassionate care for patients and families, including those experiencing end‑of‑life needs Strong communication and interpersonal skills Sound judgment within CNA scope of practice Independence and accountability in patient homes Ability to manage stress in emotionally demanding situations

Pay:

$18.61 - $21.40 per hour
