Job Description

Responsible for the effective implementation and management of services to eligible clients of the Wyoming In-Home Services (WYHS) program. Ensures program is managed in compliance with Wyoming Department of Health, Aging Division rules and regulations and all Community Action of Laramie County, Inc., (CALC) agency policies and procedures.
Job Responsibilities ACC:
The following information is intended to be representative of the essential functions performed by personnel in this position and is not all-inclusive. The omission of a specific task or function will not preclude it from the duties of this position if the work is similar, related or a logical extension of the position. Other duties may be assigned.
  • Conduct outreach activities to seek out and identify eligible clients that are 18 and over who are at risk of pre-mature institutionalization in the community. Coordinate outreach activities with existing Older Americans Act outreach service contract entities.
  • Develop and implement a system for disseminating information and educating the public regarding the needs and issues faced by family, as well as introduce the services provided through CALC's WYHS program.
  • Be a resource for client's issues in the community.
  • Receive referrals of clients from the community and partners, determine eligibility for services. Develop a written service plan for eligible clients.
  • Enter client data records for client services into three database systems by the deadlines set forth by the grant.
  • Generate and maintain accurate records of services, benefits, and other professional resources throughout Laramie County and the State of Wyoming.
  • Create and maintain working partnerships with other agencies and organizations that provide services to support the WYHS program. Serve as a liaison to the community on related committees, coalitions, etc.
  • Provide or arrange for the provision of educational opportunities that promote the program and help individuals identify themselves as caregivers.
Topics may include, but are not limited to: normal aging, chronic conditions, caregiving skills, caregiver self-care, home care products, home modifications, adaptive aids, caregiver advocacy (on behalf of the person for whom care is being provided) and other common issues facing this population.
  • Provide current information on opportunities and services available through the Wyoming Aging Network, and other service providers as appropriate. Assess the problems and capacities of clients; link WYHS clients with providers to achieve optimum opportunities and services available. Establish and implement adequate follow-up procedures.
  • Provide case management and care coordination in circumstances where family caregiver providers are experiencing diminished functioning capacities, personal conditions, or other characteristics which may require the provision of formal services by a qualified service provider (QSP).
Activities of case management include assessing needs, developing Caregiver Support Program Service Plans, authorizing and arranging services, coordinating the provision of services among providers, and follow-up and reassessment as required.
  • Make home visits or arrange for visits in a location convenient for the caregiver; complete individual WYHS client assessments on all eligible clients using the State approved forms and tools as directed.
  • Coordinate support services for the WYHS clients in the areas of health, nutrition, and financial literacy; and in making decisions and solving problems in relation to their care plan.
  • Assist in securing an appropriate level of care, for eligible clients, to temporarily give relief from the stresses and demands associated with daily 24-hour care, or for emergencies. Assist in securing supplemental services, for eligible client providers, on a limited basis to complement the care provided by caregivers.
  • Identify and arrange for payment for qualified professionals to complete individualized training or to address the needs of eligible clients.
Whenever possible, arrange training to be held in the home where care is being provided. Locate resources and individuals in the community that provide services in the areas of client stress and coping; end of life issues/grief counseling; family relations/dynamics; substance abuse; decision-making and problem-solving.
  • Receive and track in-kind donations.
  • Assist Program Director or Designee in preparation and management of relevant grants by gathering research, preparing statistical reports, etc.
  • Prepare and submit a monthly board report and other periodic reports as required.
  • Maintain an adequate inventory of supplies and program materials. Participate in regular staff meetings and trainings.
  • Provide and conduct training for WYHS Homemaker personnel.
  • Establish annually, a cost-sharing sliding fee scale for those individuals that meet or exceed the Federal Poverty Guidelines of 125% using any allowable grant or private funding.
  • Set a reasonable fee-based cost schedule annually for those individuals that do not met the Federal Poverty Guidelines.
  • Under the direction of the supervisor, supervise the WYHS Homemaker personnel. CNA/Homemaker
  • Provide personal care services as outlined in the Wyoming Home Services Grant (WyHS) contract, adhering to state policies.
  • Assist with bathing, transferring, grooming, feeding, ambulating, oral hygiene, and skin care.
  • Follow the State Policy Manual for providing in-home CNA services.
  • Routinely provide housekeeping services to dependent adults in the home setting.
  • Provide errand or chore services for dependent adults in the home setting.
  • Provide respite services for clients not needing personal care.
  • Evaluate and react appropriately to client needs, likes, and dislikes.
  • Report all concerns related to the health and safety of clients to the supervisor immediately.
  • Maintain timely and accurate communication and documentation.
Education / SKILLS
REQUIREMENTS
  • High School Diploma, Associate or Bachelor Degree
  • Experience in community development and networking, beneficial.
  • Completion of a training curriculum identified and provided by Aging Division, required.
  • Experience as a family caregiver provider, or experience working with kinship families is highly desirable.
  • Professional experience in providing social model case management, preferred.
PHYSICAL & ENVIRONMENTAL REQUIREMENTS
  • The work environmental characteristics described here are representative of typical employee encounters while performing the essential functions of this position; o May regularly sit for periods up to two (2) hours; o Use hands and fingers for a variety of grip, carry and assessment purposes; o May be required to reach with both hands and arms; and o May be required to stand, walk, climb, balance, stoop, kneel, crouch or crawl.
  • Must be able to work in a moderate noise level environment;
  • Must be able to tolerate an air conditioned environment with fluorescent lighting;
  • Be able to occasionally lift or move up to 25 pounds;
  • Be able to occasionally stand for long periods of time;
  • Be able to focus up close and routinely work with computers;
  • Be able to effectively hear, understand and communicate in varied settings, both orally and in writing;
  • Ability to speak, read and write Spanish is a plus, but not required.

  • Must possess the ability to prioritize tasks and develop effective solutions to plan and coordinate delivery of services;
  • Maintain client confidentiality in accordance with the policies of CALC and HIPAA mandates;
  • Must submit to and pass criminal background screening;
  • Must possess a valid state driver's license and have a clean driving history;
  • Must maintain and possess at least vehicle liability insurance;
  • Must be insurable through CALC's insurance provider;
  • Must have and maintain reliable transportation for job related travel;
  • Must be willing to travel within the county as needed to fulfill job responsibilities;
  • Possess general computer and word processing skills and be willing to learn other skills as required for specialized software, such as Microsoft Office Word, Excel, Outlook, Access, Power Point and Publisher;
  • Must pass all drug and alcohol screening, including pre-employment testing and random drug screens, supporting CALC's drug and alcohol free environment policies;
