Job Summary We are seeking a dynamic and compassionate Lead Care Manager to lead our care team in delivering outstanding resident-centered services. This vital role involves overseeing daily operations, managing staff, ensuring compliance with healthcare regulations, and maintaining the highest standards of patient care. Responsibilities Lead and supervise nursing staff and care teams to ensure delivery of compassionate, personalized care aligned with residents' needs. Oversee clinical assessments, develop individualized care plans, and monitor ongoing health status of residents. Ensure compliance with all state healthcare regulations, HIPAA privacy standards, and long-term care policies. Coordinate with physicians, families, and interdisciplinary teams to facilitate effective discharge planning and medical documentation. Maintain accurate medical records using EMR (Electronic Medical Records) systems such as Cerner or eClinicalWorks; ensure documentation meets NCQA standards. Requirements Proven experience in senior or long-term care settings with a strong background in nursing leadership or management. Extensive knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and healthcare regulations including HIPAA and long-term care laws. Strong supervisory skills with experience managing multidisciplinary teams in assisted living or nursing home settings. Ability to perform comprehensive patient assessments including vital signs, physical examinations, and diagnostic evaluations. Valid nursing license with current Basic Life Support (BLS) certification; additional certifications such as IV insertion or DME are advantageous.
