Aging Life Care ManagerPosition Summary Riverside Care Advisors' mission is to enhance the well-being of older adults and their families through affordable, accessible, and comprehensive care management. We are seeking a compassionate, experienced professional to join our team to provide expert guidance and care management services to older adults and their families. The Aging Life Care Manager serves as a trusted advisor, helping families navigate the complexities of aging through education, advocacy, assessment, planning, and ongoing support. This is a part-time, primarily remote/on-the-road position that could grow to full-time role. This role includes travel throughout the North Shore of Massachusetts, including Cape Ann, conducting routine visits to private homes, hospitals, rehabilitation centers, assisted living communities, skilled nursing facilities, and other client locations. Occasional evening or weekend work may be required at times based on client needs. Essential Responsibilities Client Assessment & Care Planning

• Conduct comprehensive in-home or virtual assessments of clients and families.
• Gather detailed medical, functional, psychosocial, cognitive, environmental, and family history information.
• Assess multiple areas including, cognitive function, mobility, home safety, activities and Instrumental Activities of Daily Living, social supports and caregiver stress
• Develop, monitor and revise, as appropriate. individualized care plans with practical recommendations.
• Present recommendations verbally, electronically, or through written reports based on family preference.

Family Guidance & Coaching Provide objective guidance to families navigating complex aging-related issues and decisions such as: care transitions, hospitalizations, rehabilitation, memory loss dementia progression, long-term care planning and/or end-of-life planning Care Coordination & Advocacy Coordinate services among healthcare providers, community agencies, home care providers, and residential communities. Advocate for clients during medical appointments, hospitalizations, rehabilitation stays, and care conferences. Resource Navigation Maintain current knowledge of various resources (private duty home care, assisted living communities, memory care, etc.). Provide referrals to qualified professionals when appropriate (attorneys, financial planners, mental health providers, hospice care etc.). Financial & Legal Awareness Maintain knowledge in various legal and financial areas (Massachusetts and/or New Hampshire health care systems, Community and Nursing Home MassHealth eligibility, Medicare etc.). Documentation & Technology

• Complete documentation/communication with client and family if applicable within 24 hours of visit/service.
• Maintain accurate client records in care management software.
• Keep records and report client expenses.
• Maintain HIPAA compliance with all documentation and communications.

Community Outreach Represent Riverside Care Advisors professionally in the community. Participate in networking events, educational programs, and marketing activities. Professional Development Must maintain required professional licensure; participate in ongoing education to remain current on aging-related best practices and participate in supervision, team meetings, and case consultations. May also be asked to obtain and maintain Care Manager Certification (CMC) or equivalent certification as approved by Riverside Care Advisors. QualificationsRequired

• Current Massachusetts licensure as one of the following: ○ Registered Nurse (RN)
• LPN considered based on experience ○ Licensed Independent Clinical Social Worker (LICSW) ○ Occupational Therapist (OT)
• COTA considered based on experience ○ Physical Therapist (PT)
• PTA considered based on experience ○ Licensed Speech and Language Pathologist
• Minimum of three years of experience working with older adults.
• Excellent written and verbal communication skills.
• Strong assessment and critical thinking abilities.
• Ability to work independently while collaborating effectively with a multidisciplinary team.
• Excellent organizational and time management skills.
• Valid driver's license, reliable transportation, and appropriate level of insurance.
• Must be able to perform the physical requirements of the job as more fully outlined in the full Job Description which include, but are not limited to sitting, standing, walking, climbing stairs, lifting and moving up to 30 pounds. Preferred
• Certification as a Certified Care Manager (CMC) or willingness to obtain certification.
• Experience in Aging Life Care™ or Geriatric Care Management.
• Knowledge of dementia care and caregiver support.
• Experience with Google Suite and Microsoft.

Pay:

$50.00 - $60.00 per hour
