General Position Overview:

ONEgeneration's Multipurpose Senior Services Program (MSSP) serves older adults who are frail and at risk of institutional placement. The objective of MSSP is to prevent premature placement of individuals in nursing facilities , while fostering independence and enhancing quality of life through coordinated care management and supportive services. The program enables eligible participants to remain in or return to their homes by providing individualized, comprehensive case management and access to community-based resources.

Position Summary:

Under the supervision of the Supervising Care Manager (SCM), the Social Work Case Manager provides community-based care management and advocacy to frail, at-risk older adults enrolled in the Multipurpose Senior Services Program (MSSP). The primary goal of this position is to prevent or delay premature institutionalization by promoting independence and enhancing quality of life through individualized care planning and linkage to community resources. This is a field-based/remote position requiring regular travel throughout Ventura County for in-home assessments, care plan monitoring, and collaboration with participants, caregivers, and service providers.

Main Responsibilities:

The major responsibilities of this position include, but are not limited to: Conduct in-home comprehensive psychosocial assessments to evaluate participants' needs, strengths, and risks. Develop individualized care plans in collaboration with the Nurse Care Manager (NCM), ensuring that plans are participant-centered and goal-oriented. Carry an active caseload of approximately 50-52 participants . Conduct initial, annual, and reassessment home visits as scheduled and required by MSSP guidelines. Coordinate, authorize, and monitor services and interventions outlined in the care plan. Maintain accurate and timely case documentation in compliance with CDA (California Department of Aging) MSSP requirements. Document all participant contacts, interventions, and case management activities on a monthly basis. Monitor and track progress toward care plan goals and adjust services as needed. Identify and strengthen formal and informal support systems , linking participants and caregivers to appropriate community-based resources. Collaborate and communicate effectively with healthcare providers, family members, vendors, and community agencies. Explore and exhaust all community and program resources before purchasing and authorizing waiver services . Maintain a monthly log of participant expenses related to waiver service authorizations. Participate in team case conferences , staff meetings, and ongoing professional development. Perform related duties as assigned to ensure the overall effectiveness and quality of the MSSP program.

Knowledge and Skills:

Demonstrated ability to exercise independent judgment , problem-solving, and critical thinking. Knowledge of elder care issues , including risk factors for elder and dependent adult abuse or neglect. Strong understanding of community resources , social services, and aging network systems. Ability to establish rapport and work effectively with frail older adults, caregivers, and families. Excellent written and verbal communication skills , including clear documentation and active listening. Strong organizational and time management skills; ability to meet deadlines and program standards . Proficiency in Microsoft Office applications (Word, Excel, Outlook) and case management systems. Ability to work effectively both independently and as part of an interdisciplinary team . Maintain strict confidentiality and adhere to professional ethics and program policies. Demonstrated cultural competence and sensitivity when working with diverse populations.

Qualifications:

Bachelor's degree in Social Work, Human Services, Psychology, Gerontology, Sociology, or a related field. Two (2) years of direct experience working with the elderly or disabled populations in a social services or healthcare setting. Bilingual, Spanish speaking preferred. Clean driving record and Driver licenses, proof of car ownership and car insurances. Negative tuberculosis (TB) screening prior to employment. Live Scan fingerprint background check clearance in compliance with state and federal regulations. Bilingual (English/Spanish) fluency strongly preferred. Must Live in Ventura county.

Preferred Qualifications:

Experience in MSSP or other waiver-based case management programs . Familiarity with CDA Waiver requirements and Medi-Cal-funded programs. Residency in Ventura County strongly preferred. Working conditions :

• Valid California driver's license , reliable vehicle, and proof of current automobile insurance . Daily driving may be required to fulfill participant and program needs.
• Negative tuberculosis (TB) screening prior to employment.
• Live Scan fingerprint background check clearance in compliance with state and federal regulations.
• Bilingual (English/Spanish) fluency strongly preferred.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position: Work performed in moderate noise levels and a non-smoking environment. Frequent sitting, standing, walking, bending, and climbing stairs during home visits and outreach activities. Occasionally lift and/or move up to 25 pounds independently. Ability to hear, speak, and communicate clearly in person and by phone or video. Regular use of a computer, keyboard, and other office equipment . Majority of job duties are performed remotely and in the field ; visits to participant homes and community partner sites are required.
