Full Job Description Working Hours :

Full Time hours 9AM TO 1PM (1 to 2 lunch) 2PM TO 6

PM Full Time:

Vacation:

40 hours per year and 40-hour sick time; accumulated at 1 hour per 30 hours worked. Ten paid holidays.

Mileage Reimbursement:

76 cents per mile from office to client location.

Key Responsibilities:

As Community Support Assistant, your primary responsibilities will include but not limited to: Visit SNF/other facilities and explain ECM program to social workers and discharge coordinator/nurse and refer patients to Sowing Seeds ECM program. As a Lead Care Manager, your primary responsibilities will include, but are not limited to: Outreaching to members to enroll, consent, and authorize services with health plan, Assessing patients in person or by phone if patient declines in person visit. Developing and implementing comprehensive care plans for individuals with complex medical needs. Each patient requires minimum 2 hours per months of services. You will carry patients for 1 year. Coordinating with healthcare providers and community resources to ensure continuity of care. Conducting regular assessments and reassessments to monitor and evaluate the effectiveness of care plans. Collaborating with interdisciplinary teams to optimize patient outcomes and quality of care. Providing education and support to patients and their families regarding their healthcare needs. Maintaining accurate and up-to-date documentation of patient interactions and care plans. Performing home visits. Accompanying patients at doctor appointments (not driving patients but meet patients at the office, or joining on zoom) Skill nursing home visits if patient is at the nursing home. Visiting primary care physician office on behalf of ECM patient Finding coverage and call forward the phone and email if you need time off.
