Responsibilities Coordinate with social workers, healthcare providers, and community agencies to ensure timely access to social services, and behavioral health support. Manage intake processes for new clients, ensuring accurate documentation and assessment of social determinants affecting health outcomes. Conduct client communication with empathy and clarity, provide education on available resources and navigate complex healthcare systems. Monitor utilization review and management activities to optimize client care pathways while adhering to HIPAA regulations and Medicaid. Support culturally competent care by working effectively with individuals from diverse backgrounds and bilingual populations. Maintain detailed records of case management activities, and documentation of social work interventions. Experience & Qualifications Bachelor's degree in social work, human services, psychology, public health, Criminal justice, counseling, healthcare administration, or a related field. Strong communication and organizational skills, and strong problem-solving skills. Ability to work independently in a remote environment. If a candidate has proven & qualified experience in case management or a related field and does not have a degree then experience will be taken into consideration. Preferred Experience in Care Management, Case Management, Social Services, Behavioral Health, Healthcare Navigation or Community Outreach. Familiarity with medical terminology, and managed care processes. Familiarity with New York State Medicaid Programs. Health Homes, Care Coordination, or Social Care Network Initiatives. Proficiency with Microsoft Office applications for documentation and reporting purposes. Prior experience working with social services teams or case management roles involving patient service coordination. Ability to communicate effectively across multidisciplinary teams while demonstrating strong client communication skills in a diverse cultural context.
