$53,000+ Incentive Bonuses - SPARC Services and Programs is seeking a Qualified Professional to join our Care Management team in the Rowan/Iredell County area. About the Role As a Care Manager, you'll provide whole-person integrated care management services to members enrolled in tailored plans. Your work will directly impact individuals who need coordinated, consistent support, and you'll be the person they count on to help navigate it. What You'll Do Working through a combination of in-person visits and phone-based support, you'll deliver a full range of care management services, including: Comprehensive assessments and individualized care plans Care team development and ongoing care coordination Transitional care management Health promotion and medication monitoring Addressing unmet health-related resource needs Promoting a system of care approach 24/7 on-call crisis support (rotational coverage) Schedule This is a full-time position with a flexible, hybrid schedule. Qualifications Bachelor's degree in a Human Services field (psychology, sociology, another relevant human services area, or licensure as a registered nurse) Minimum 2 years of experience working directly with individuals with mental health, substance use, or other behavioral health conditions Valid driver's license and reliable transportation Join a diverse, inclusive, and whole-person focused team!
