B&D Integrated Health Services is seeking a full-time Tailored Care Manager . Under the direction of the Tailored Care Management Supervisor, the Tailored Care Manager is responsible for providing medical care management services to a specified panel of patients within the clinical setting. The primary responsibilities of the Care Manager are to promote the whole person, and integrated care by planning, coordinating, tracking, closing gaps in care, and monitoring care to individuals. The Care Manager facilitates the integration of physical health, behavioral health, and community supports through an intentional and ongoing engagement of multidisciplinary providers. The Care Manager actively engages with members by ensuring the delivery of comprehensive assessments and care management, care coordination, health promotion, transitional care, individual and family support, and referrals to community and social support services. TCM Services can take place face-to-face, telehealth, or via phone. What is Tailored Care Management? TCM is a service designed to connect clients with local providers/resources which will address their needs as it relates to their physical and mental health and environment while promoting self-sufficiency and independence. What is the goal of Tailored Care Management? To assist clients with coordinating care within ALL aspects of life to improve their individual functionality and reduce dependence on physical and mental providers. Tailored Care Manager is solely responsible for: Completing the Care Management Comprehensive Assessment. Developing the Care Plan/ISP Facilitation of Case Conferences Ensuring that medication monitoring and reconciliation occur Continuous monitoring of progress toward the goals identified in the care plan/ISP Managing care transitions, including 90-day transition plans
THIS POSITION OFFERS A 40-HOUR WORKWEEK, NO WEEKENDS, FULL BENEFITS PACKAGE, 12 PAID HOLIDAYS, BI-WEEKLY PAY.
Experience Requirements / Minimum Qualifications Bachelor's degree in a field related to health psychology, sociology, social work, nursing, or another relevant human services area -- combined with two years of experience working with individuals with behavioral health conditions. Bachelor's degree in a field other than human services combined with four years of full-time, post-bachelor's degree experience working with individuals with behavioral health conditions is also acceptable. Effective communication and interpersonal skills. Must demonstrate the skills and judgment necessary to provide direct care to patients under the direct supervision of the Supervising Tailored Care Manager. Strong computer skills and working knowledge of the windows-based environment and Electronic Health Record (EHR) experience.
