This vital role involves coordinating comprehensive post-incarceration health and social services to support individuals reentering the community. The Reentry Care Manager will serve as a bridge between healthcare providers, community resources, and clients, ensuring seamless continuity of care. This position offers an exciting opportunity to make a meaningful impact by promoting recovery, reducing recidivism, and fostering healthier lives through personalized case management and resource coordination. Duties Conduct thorough intake assessments to evaluate clients' medical, mental health, substance use, and social needs upon reentry. Develop individualized care plans that incorporate medical treatment, behavioral health support, addiction counseling, and social services. Manage electronic health records (EHR) to document client progress, treatment plans, and service delivery while ensuring compliance with HIPAA regulations. Collaborate with community organizations to connect clients with housing assistance, employment programs, mental health services, addiction treatment, and other social supports. Provide patient education on physiology, medical terminology, medication adherence, and healthy lifestyle choices tailored to diverse cultural backgrounds. Engage in ongoing case management activities such as diagnostic evaluation, client communication, behavioral health interventions, and utilization management to optimize patient outcomes. Prioritize tasks effectively in a fast-paced environment involving inpatient stays, outpatient visits, and post-acute care follow-ups. Required Qualifications Active Washington State license as an RN, LCSW, or LMHC. Experience working with individuals involved in the criminal justice system, behavioral health, substance use, or care coordination preferred. Strong assessment, documentation, communication, and care coordination skills. Ability to work collaboratively with medical, mental health, SUD, and reentry teams. Ability to maintain confidentiality and work professionally in a correctional setting. Must pass an extensive background check. Key Responsibilities Complete comprehensive reentry health assessments addressing physical health, behavioral health, substance use, mental health, and health-related social needs. Identify medical, behavioral health, housing, transportation, employment, education, and other reentry needs. Develop individualized Reentry Care Plans based on the patient's goals, health conditions, and identified needs. Identify and coordinate referrals for medical, mental health, SUD, housing, transportation, and community-based services. Support treatment engagement and assess the patient's readiness to participate in treatment. Coordinate with community providers and reentry partners prior to release. Assist with continuity of medications and connection to community providers. Identify barriers to successful reentry and assist with addressing those barriers. Maintain accurate and timely documentation of assessments, care plans, referrals, and follow-up. Update care plans when there are significant changes in the patient's health, circumstances, or release date. Participate in multidisciplinary care coordination and discharge planning.
