Job Overview We are seeking a dedicated and energetic Licensed Practical Nurse (LPN) to step into the vital role of Charge Nurse within our dynamic healthcare team. In this position, you will lead nursing staff, coordinate patient care, and ensure the delivery of high-quality medical services. Responsibilities Oversee daily nursing operations, ensuring adherence to clinical protocols and safety standards in compliance with healthcare regulations such as HIPAA and NCQA standards Supervise and support nursing staff, providing guidance on patient care procedures and fostering professional development Coordinate patient assessments, vital signs monitoring, and medical documentation using EMR (Electronic Medical Records) systems Administer medications, injections, and perform catheterizations following physician orders Conduct physical examinations, evaluate patient conditions, and participate in diagnostic evaluations, including laboratory specimen collection and processing Collaborate with interdisciplinary teams on discharge planning, case management, and long-term care strategies for diverse patient populations including geriatrics, dementia care, and individuals with developmental disabilities Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification required Proven experience in long term care facilities Hands-on experience with EMR/EHR systems Excellent patient assessment skills Strong communication skills Knowledge of HIPAA regulations to ensure confidentiality and proper handling of medical records Join us in making a meaningful difference by providing compassionate care that improves lives.
