- $44 an hour 2 hours ago Qualifications Computer operation RN License English Managing patients as a nurse case manager Driver's License Bachelor's degree Data management Bachelor's degree in nursing Computer skills Full Job Description Under the supervision of the Multipurpose Senior Services Program (MSSP) Site Director/Supervising Care Manager, the Registered Nurse Care Manager (RNCM) functions as a geriatric care manager for frail low-income elders enrolled in the MSSP Medi-Cal Waiver program.
Essential Duties:
Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions. Screen applicants to MSSP using established criteria and screening forms to determine level of care appropriate for MSSP. Conduct Initial Health Assessment- in participant's home or via phone.
Minimum Qualifications:
Bachelor's Degree in Nursing (public health, health education, health administration, gerontology, or equivalent). 1-2 years of clinical nursing experience, preferably with frail older adults or one year of case management experience. Hold a valid current license to practice nursing in the State of California, a public health certificate issued by the State of California is a plus. RNCM must have the ability to communicate in English in both written and verbal forms. Must have valid California Driver's license, clean DMV record, and automobile insurance. The RNCM must possess skills to manage care for vulnerable crisis-prone participants with diverse medical and social problems. Strong computer literacy to work in an electronic data management system and Excel. Current first aid/CPR certification required. Must pass Department of Justice fingerprint clearance.Please Note:
