Unit Manager-LPN/RN Full Time Hamden Rehabilitation and Healthcare Center We are seeking a dedicated and experienced Unit Manager-LPN/RN to join our team at Hamden Rehabilitation and Healthcare Center. As a key member of our leadership team, you will be responsible for providing exceptional care and management to our residents.
Job Summary:
The primary purpose of this position is to provide clerical support at the nurse's station, ensuring seamless communication and efficiency in our daily operations. You will be responsible for maintaining accurate records, scheduling appointments, and providing administrative support to our nursing team.
Job Responsibilities:
Deliver new admission charts to the Unit and initiate forms in preparation for admission Fax physician's orders daily and deliver printed copies to the appropriate department Complete daily transfer of vital signs and weights from the daily assignment sheet to the resident's chart Maintain an orderly and neat nurse's station Screen all incoming phone calls for nursing and forward messages Schedule appointments with physicians for residents and relay information to nursing staff Initiate phone calls to physicians, family members, or personnel as needed Input data, including daily entry of labs, new admission information, and updates to resident information Notify other departments of applicable changes and maintain a current bathing schedule on the bath record Greet visitors, guests, customers, and provide information/directions in accordance with company policies and procedures Ensure residents and families receive exceptional care and service in a caring and compassionate atmosphere
Job Skills:
Strong organizational ability Ability to handle a variety of assignments, some of moderate complexity, and requiring considered familiarity with the operating details of the Center Ability to interpret and apply departmental procedures Courteous and tactful at all times Ability to operate a computer (PointClickCare, Windows Office, etc.) Ability to demonstrate discretion in handling confidential information (HIPPA)
Working Conditions:
Works throughout the nursing service area, including drug rooms, nurses' stations, resident rooms, etc. Moves intermittently during working hours Subject to frequent interruptions May be subject to hostile and emotionally upset residents, family members, or visitors Communicates with nursing personnel and other Center personnel May be required to work beyond scheduled working hours, including weekends and holidays
Schedule:
Monday through Friday 11AM-7PM.
Salary:
LPN-$36 to $40/Hour RN- $43 TO $48/
Hour Licenses:
Must have a valid LPN or RN license in the state of CT.
