As we continue to grow our Home Health services throughout South Florida, we are seeking an experienced and compassionate RN Clinical Manager to provide leadership in case management, clinical oversight, and staff support. This role is ideal for a Registered Nurse who thrives in a collaborative environment and is passionate about mentoring clinicians, coordinating patient care, and ensuring outstanding clinical outcomes. About the Role The RN Clinical Manager is responsible for overseeing the day-to-day clinical management of Home Health patients and supporting the performance and development of field clinicians. This position ensures patients receive high-quality, coordinated care while maintaining compliance with Medicare Conditions of Participation, state regulations, and agency policies. The Clinical Manager serves as a clinical resource for the interdisciplinary team, providing guidance on patient care, case management, documentation, and regulatory compliance. What You'll Do Case Management Oversee a caseload of Home Health patients and ensure appropriate case management practices. Review and coordinate admissions, recertifications, resumptions of care, transfers, and discharges. Ensure individualized plans of care are developed, implemented, and updated based on patient needs. Collaborate with physicians and the interdisciplinary team to coordinate timely, effective patient care. Monitor patient progress and intervene when changes in condition or barriers to care arise. Clinical Staff Oversight Supervise and support field clinicians, including RNs, LPNs, therapists, social workers, and home health aides. Serve as a clinical mentor and resource for staff regarding patient care, documentation, and best practices. Conduct case conferences and provide ongoing clinical guidance. Participate in orientation, competency validation, and ongoing education of clinical staff. Assist with performance coaching and development to promote clinical excellence. Quality & Compliance Review clinical documentation for accuracy, completeness, and regulatory compliance. Ensure compliance with Medicare Conditions of Participation, state regulations, accreditation standards, and agency policies. Monitor quality indicators and support performance improvement initiatives. Work collaboratively with Quality Assurance to improve clinical outcomes and documentation quality. Promote patient safety and adherence to evidence-based practices. Care Coordination Collaborate with Intake, Quality Assurance, and Operations to ensure seamless patient care. Assist with clinician caseload balancing and appropriate patient assignments. Support efficient communication between field staff, physicians, referral sources, and internal departments. Help resolve clinical issues and facilitate timely patient care decisions. What We're Looking For Active Florida Registered Nurse (RN) license in good standing. Minimum of 3 years of Home Health nursing experience required. Minimum of 1 year of experience as a Clinical Manager, Case Manager, or clinical supervisor. Strong knowledge of Medicare Conditions of Participation and Home Health regulations. Experience with OASIS documentation and Home Health case management. Excellent clinical assessment and critical thinking skills. Strong leadership, communication, and organizational abilities. Ability to mentor and support multidisciplinary clinical teams.
