Job Overview We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) to serve as a Charge Nurse within our healthcare facility. As a Charge Nurse, you will oversee daily nursing operations, coordinate patient services, and mentor nursing staff to ensure compliance with healthcare standards. Responsibilities Supervise and coordinate nursing staff during shifts, ensuring efficient and effective patient care delivery. Assess patient conditions through comprehensive physical examinations and vital sign monitoring, utilizing physiology knowledge to inform care plans. Administer medications, including injections and IV infusions, adhering to HIPAA regulations and safety protocols. Oversee medical documentation using EMR (Electronic Medical Records) systems such as Epic or Cerner, ensuring accuracy and compliance with healthcare standards. Manage patient assessments, triage urgent cases, and prioritize care based on acuity levels in various settings including acute care, long-term care, or assisted living environments. Collaborate with multidisciplinary teams for discharge planning, case management, and coordination of post-acute or hospice care services. Ensure infection control practices are maintained rigorously to prevent the spread of infectious diseases within the facility. Provide education on health coaching topics such as nutrition, medication management, and disease prevention tailored to individual patient needs. Requirements Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction. Proven experience in critical care settings such as ICU or Level I Trauma Centers is highly desirable. Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth; experience with

ICD-9/ICD-10

coding is preferred. Strong background in diverse clinical areas such as medical-surgical nursing, geriatrics, dementia care—including Alzheimer's and memory care—and behavioral health. Experience working with individuals with developmental disabilities or those requiring specialized care such as tube feeding or ventilator management. Knowledge of infection control protocols, sterile techniques, and aseptic procedures. Excellent assessment skills combined with the ability to manage acute pain and perform diagnostic evaluations effectively. Ability to work collaboratively within a team environment while demonstrating leadership qualities essential for charge nurse responsibilities. Additional skills in phlebotomy, IV insertion, catheterization, suturing, or spinal tap procedures are advantageous but not mandatory.
