Job Summary We are seeking an energetic and detail-oriented Patient Care Manager to lead our clinical team in providing outstanding care to patients across various settings. This vital role involves overseeing patient assessments, coordinating multidisciplinary care plans, ensuring compliance with healthcare regulations, and fostering a supportive environment for both patients and staff. Duties Lead and supervise nursing staff, ensuring adherence to nursing policies and standards such as HIPAA and long-term care regulations Coordinate patient assessments, including vital signs, physical examinations, and medical documentation using EMR (Electronic Medical Records) systems like Cerner or Athenahealth Oversee care planning for diverse populations including geriatrics, pediatrics, behavioral health, dementia care, and individuals with developmental disabilities or long-term conditions such as dialysis or ventilator dependence Manage utilization review and utilization management processes to optimize patient outcomes while complying with state healthcare regulations and DRG (Diagnosis-Related Group) coding standards Facilitate discharge planning, case management, and family education to ensure seamless transitions across care settings such as hospitals, nursing homes, assisted living facilities, or home care environments Ensure all patient care activities incorporate infection control protocols, sterile processing techniques, aseptic methods, and proper specimen collection/processing procedures Support team development through supervising experience, managerial communication, nursing leadership initiatives, and ongoing staff training in areas like IV infusion, catheterization, tube feeding, and basic life support Skills Extensive knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques Proficiency with EMR/EHR systems; familiarity with
ICD-9/ICD-10
coding and CPT coding for accurate medical documentation Critical care experience including ICU or Level I & II trauma center background; experience working with acute pain management and ventilator management preferred Strong leadership skills in supervising nursing teams within hospital medicine or long-term care environments; adept at managing multidisciplinary teams in high-pressure situations Ability to perform comprehensive patient assessments including vital signs, venipuncture (phlebotomy), injection administration, catheter care, and sterile techniques Knowledge of healthcare regulations including HIPAA compliance and state-specific healthcare laws governing long-term care facilities Excellent communication skills for effective managerial coordination; experience working with individuals with disabilities or complex health needs highly valued More About Us Sacred Encounter is a World-Class healthcare organization where every aspect of the care experience is considered important and sacred.
