Job Description

Resident Care Coordinator Clinical Leadership and Care Staff Management We are seeking a dedicated and experienced Resident Care Coordinator to manage the day-to-day coordination of direct patient care within our community. This key position involves hands-on care, staff management, clinical partnership, and ensuring compliance with all resident care standards. Reporting to the Executive Director, the ideal candidate is a compassionate leader who excels at staff development, resident care planning, and maintaining a high level of customer satisfaction. The Resident Care Coordinator is primarily responsible for coordinating the direct patient care of all residents. This includes hiring, training, developing, evaluating, scheduling, and supervising all care staff across all shifts, ensuring optimal care delivery and adherence to care plans. Essential Functions Provide leadership, training, direction, and development opportunities to care staff. Participate in hiring, setting clear expectations and goals, and managing the disciplinary process as needed. Direct staff in daily responsibilities to promote company loyalty and success. Responsible for the safety of all people in the facility, potentially assigning a Safety Officer to assist. Partner with the facility nurse to coordinate resident care, train staff on care plans, and communicate any change of conditions. Manage and ensure the implementation of all resident care plans, ensuring staff are appropriately trained and scheduled to meet resident needs. Directly assist residents with Activities of Daily Living (ADLs), including grooming, hygiene, dressing, eating, ambulating, medication assistance, toileting, and turning/positioning residents (requiring lifting up to 35 pounds). Utilize therapies and techniques to prevent and improve resident behaviors. Develop and maintain relationships with residents, families, staff, and outside agencies. Ensure customer satisfaction by effectively handling complaints, grievances, and concerns. Participate in the development and implementation of strategic plans to increase efficiency and growth. Partner with Senior Care Advisors to promote the facility, determine appropriate candidates for admission, and ensure the facility is tour-ready. Assist with debt collection and payment of debts. As necessary, perform the work required in any of the positions at the facility (e.g., cooking, cleaning, and housekeeping). May transport residents, as needed.
Job Specifications Age:
Must be at least 21 years of age.
Experience:
A minimum of 1 year of experience in health care or a related field is required.
Supervisory Experience:
Previous supervisory experience is preferred.
Certifications:
Certifications in First Aid, Cardiopulmonary Resuscitation (CPR), and medications are required. Certified Nursing Assistant (CNA) preferred.
Knowledge:
Knowledge of state rules, regulations, fire codes, safety practices, residents' rights, purchasing/budgeting procedures, and financial statements.
Skills:
Ability to supervise and motivate staff, excellent verbal and written communication skills, computer proficiency (Word, Excel, Email), and ability to maintain confidentiality. Other Requirements Must possess valid drivers license and reliable transportation. Must pass a criminal background check. Must complete continuing education required by the state licensing agency. Work Environment and Physical Demands Work is performed in both an office and a health services environment. May be required to work irregular hours including nights, weekends, and on-call availability. Must be able to lift up to thirty-five (35) pounds unassisted. May be required to travel for meetings, trainings, or to support other facilities. #
