Current RN Licensure in the state of Ohio BA in nursing preferred/ Associates required 2 to 3 years clinical practice experience as an RN Well-rounded clinical experiences including work in outpatient settings as an RN Experience with data analysis and health care operations A day in the life of aHSO Care Manager:
Assists patients through the healthcare system by acting as a patient advocate and navigator. Coordinates continuity of patient care with patients and families following hospital admission, discharge, and ER visits. Educates patient and family on ways to reduce ER visits in the future Creates and support Chronic Care Plans Participates in Patient-Centered Medical Home team meetings and quality improvement initiatives. Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.Who We Are:
