We're looking for an experienced and compassionate MDS Coordinator to join our clinical team and help ensure each resident receives individualized, high-quality care. Compensation & Benefits $37-$44/hour , based on experience and qualifications Full-time, Monday through Friday Day shift 401(k) Health, Dental, Vision & Life Insurance Disability Insurance Paid Time Off (PTO) & Paid Holidays Professional Growth & Development Opportunities What You'll Do Complete and manage Minimum Data Set (MDS) assessments accurately and on time Coordinate and guide individualized resident care plans Collaborate with nursing, therapy, residents, families, and interdisciplinary team members Monitor resident progress and update care plans as needs change Ensure documentation supports resident care, regulatory requirements, and reimbursement Educate and support staff on care plans and MDS-related processes Maintain accurate records and communicate updates with residents and families Work with healthcare providers and outside agencies to coordinate care as needed What We're Looking For Current and active RN license preferred; experienced LPNs encouraged to apply MDS, skilled nursing, long-term care, or related clinical experience preferred Strong understanding of resident assessments and individualized care planning Excellent communication and organizational skills Ability to work collaboratively with residents, families, and interdisciplinary teams Compassionate, professional approach to resident care Ability to work independently and manage deadlines effectively Bachelor's degree preferred Schedule & Location Full-Time Monday-Friday Day Shift In-person at our Bensenville, IL campus Why Bridgeway Senior Living?
