Job Description

The Heathers Senior Homes is a family-owned, boutique assisted living and memory care community with homes in Lake in the Hills and Ringwood, Illinois. We are proud to have been recognized as McHenry County's Best Senior Living Community for three consecutive years. We believe senior living should feel like home. Our intimate communities are built around meaningful relationships, personalized care, and an environment where residents can continue to live with purpose, dignity, independence, and connection. We are committed to doing Assisted Living & Memory Care — The Heathers Way. About the Role We are looking for a Clinical Manager to join our leadership team at our Ringwood home. With a maximum of just 9 residents , this is a unique opportunity for a nurse who values relationships, individualized care, and being truly involved in the day-to-day lives of the residents they serve. This is a hands-on clinical leadership role . The Clinical Manager will provide nursing care, support and develop the care team, oversee clinical needs, and partner closely with the Director of Nursing and Executive Director to ensure residents receive exceptional, safe, and individualized care. The right person will be more than a clinical resource — they will be a trusted leader, problem-solver, advocate, and familiar face for residents, families, and team members. What You'll Do Provide hands-on nursing care and clinical oversight while supporting the daily operations of the home. Partner closely with the Director of Nursing to ensure consistent, high-quality resident care. Oversee assessments, care plans, medication management, physician orders, documentation, and follow-up on changes in condition. Lead, coach, and support Care Partners and nursing team members, reinforcing high standards for resident care, safety, documentation, and infection prevention. Build meaningful relationships with residents and families and respond to concerns with compassion, urgency, and professionalism. Coordinate with physicians, nurse practitioners, pharmacies, hospice, home health, therapists, and other healthcare partners to ensure continuity of care. Support resident admissions, assessments, care planning, and transitions of care. Support IDPH compliance, survey readiness, quality improvement, incident follow-up, and clinical education. Provide education and ongoing support to team members, including dementia care and other clinical competencies. Partner with leadership to identify opportunities to improve resident care, team performance, and the overall resident experience. Participate in a 24/7 on-call rotation and provide clinical leadership when urgent resident or operational needs arise. Jump in wherever needed — because at The Heathers, leadership means being willing to roll up your sleeves and do what is best for our residents and team. What We're Looking For Active Illinois RN or LPN license. Current CPR certification. Nursing experience in assisted living, memory care, long-term care, home health, or a related setting preferred. Previous clinical leadership or supervisory experience preferred. Knowledge of IDPH requirements, clinical documentation, assessments, care planning, and medication management preferred. Strong clinical judgment and the ability to remain calm and confident in changing situations. A natural leader who enjoys coaching, developing, and supporting others. Excellent communication and relationship-building skills. Someone who is energetic, compassionate, accountable, and hands-on. A genuine passion for seniors and a belief that exceptional care starts with knowing the person behind the care plan. Ability to participate in a 24/7 on-call rotation. Why The Heathers? At The Heathers, leadership is personal. We believe our residents deserve to be known — not just by their diagnoses or care plans, but by their stories, personalities, preferences, and what makes them who they are. We believe families should feel confident in the care their loved one receives. And we believe employees should feel supported, respected, and empowered to do meaningful work. If you are a hands-on nursing leader who loves people, believes in individualized care, and wants to make a meaningful difference in the lives of seniors and the team caring for them, we'd love to meet you. Join us and help us do Assisted Living & Memory Care — The Heathers Way.
