Nursing Services Supervisor
Job Description
Clinical Operations Responsible for the day-to-day operations of the designated region to ensure agency goals and superior patient care are achieved. Manages and directs a team of clinicians, including RN Case Managers, RNs, LPNs, Social Workers, and Home Health Aides, to ensure safe, effective, and appropriate home care services are provided. Functions as the primary resource for associate questions and concerns and provides guidance regarding proper patient care. Meets with clinicians regularly on an individual basis to provide guidance and information related to specific issues. Collaborates with the QA department and provides education to staff regarding OASIS corrections and chart revisions. Provides direction to teams to ensure client needs are met and services are provided according to agency policies and procedures. Educates clinicians on clinical services, policies, and procedures, as needed. Serves as the clinical expert on home health guidelines and Medicare regulations to accurately respond to associate concerns and questions. Ensures the accuracy, completeness, and timeliness of clinical documentation in accordance with agency policies and procedures, regulatory requirements, and industry standards. Performs weekly reviews of clinician productivity and reports findings to the Area Manager. Coordinates 24-hour on-call coverage and acts as a backup to provide patient care, if needed. Reviews on-call logs and provides summaries of reasons for calls and resolutions. Leads weekly case conferences to review admissions, discharges, recertifications, potential LUPAs, patient outcomes, team SHP scores, and other patient-specific needs. Ensures discharge planning is completed and required documentation is assigned and/or returned for upload to the chart. Conducts ongoing chart and clinician reviews, including: New admission reviews Frequency orders Scheduled supervisory visits Wound care Visit exceptions (overlapping visits and visits less than 15 minutes) Outstanding visits (past due visits) Mileage reviews for excessive mileage Conducts ongoing nurse meetings to perform in-services and promote educational training based on the Education Plan and identified regional skill needs. Personnel Management Reviews employee timecards/payroll daily via the HRIS to monitor overtime, pay premiums, on-call, absenteeism, and ETO requests. Follows agency policy regarding ETO requests to ensure adequate patient coverage. Collaborates with Human Resources regarding policy/procedure violations or performance concerns to deliver performance improvement plans and corrective actions. Assists in screening and interviewing new agency personnel and makes recommendations to the Director of Clinical Operations and Human Resources. Assists with new hire orientation and ensures precepting is completed to agency expectations. Performs annual observation visits with clinicians to determine staff competency and respond to educational and developmental needs. Completes performance appraisals in a timely manner to deliver effective feedback for success. Staffing Oversight Performs staffing oversight to ensure adequate patient coverage in the designated region. Promotes self-scheduling for field staff and performs a high-level overview to plan for productivity. Prioritizes referrals based on acuity and timely initiation of care requirements. Collaborates with the Area Manager to ensure territories are assigned appropriately to achieve adequate staffing coverage. May perform screening calls to hospital and physician office referrals to ensure patients are at home, available, and appropriate for admission, and to confirm patient availability. Collaborates with Intake regarding scheduling of new admissions and evaluations. Maintains consistent communication with the Intake department, Marketing department, and Area Manager regarding staff availability. Partners with the Area Manager regarding staff vacations and call-ins to ensure patients are seen in compliance with physician-ordered frequency. Collaborates with the Intake department daily to review staffing needs, properly serve patient census, and assist clinicians with self-scheduling as needed. Direct Patient Care Provides skilled nursing services in accordance with adult and geriatric patients' plans of care. This includes comprehensive evaluations, ongoing monitoring of home health services, and education and training for patients, caregivers, and family members. Completes initial and ongoing assessments to establish an individualized home health plan of care with the physician. Administers nursing procedures and treatments as indicated by the physician to ensure patients achieve desired outcomes while receiving home health services. Communicates with the physician regarding patient needs, reports changes in patient condition, and obtains physician orders as needed for revisions to the plan of care.
