Position Summary:

Under the supervision of Fitness Director and the clinical oversight of the Medical Director of Cardiology, the Cardiac Rehab Clinical Coordinator provides comprehensive patient assessment, orientation, and care coordination services within the Cardiopulmonary Rehabilitation Program. The role ensures the delivery of individualized, evidence-based care plans designed to improve cardiovascular health, promote lifestyle modification, and enhance overall patient well-being through exercise prescription, education, nutrition guidance, and ongoing outcome evaluation.

Responsibilities:

Provides comprehensive patient assessment, education, and care coordination for individuals with coronary artery disease (CAD), peripheral artery disease (PAD), and chronic pulmonary conditions. Initiates and/or revises an individualized treatment plan that includes immediate and long-term outcomes for the patient/family and reflects the plan of the entire health care team. Monitors patient/member and reports any inappropriate responses to exercise and terminates exercise as needed; interprets EKGs and complex arrhythmia; maintains accurate documentation of intervention due to critical deviations of plan of care; and assists with emergency care according to standing orders. Responsible for the ongoing operation of the cardiac rehabilitation program; develops, implements, and reviews program protocols, standing orders, develops admissions process, exercise guidelines, emergency protocols, staffing patterns, record keeping routines, policies and procedures with Medical Director. Builds and develop new programs and processes as national trends in Cardiopulmonary Rehabilitation evolve. Educates patients and families on cardiovascular and pulmonary disease processes, treatment options, and self-management strategies. Identifies, monitors, and addresses modifiable risk factors, including tobacco use, hypertension, hyperlipidemia, diabetes, obesity, physical inactivity, and stress. Promotes evidence-based lifestyle modifications to reduce disease progression, prevent complications, and improve long-term health outcomes. Utilizes current clinical guidelines and best practices in the prevention, treatment, and rehabilitation of cardiopulmonary and vascular disease. Collects and monitors program outcomes to evaluate areas of need for program quality improvement projects.

Qualifications:

Required:

Graduate of an accredited school of nursing Current Registered Nurse (RN) licensure in the State of Kansas or has work permit from the State Board of Nursing Basic Life Support (BLS) Automated External Defibrillator (AED) and Basic First Aid certification required or must complete BLS and AED certification with First Aid Certification in the first two weeks of employment Advanced Cardiac Life Support (ACLS) and Basic First Aid certification to be completed within the first six months after hire. Knowledge of general nursing theory and practice

Infection Control:

Initial and Ongoing training in dealing with infection control. Trainings could include but are not limited to, blood borne pathogens, bodily fluids and bio hazardous materials as it applies to your daily work environment.
