In this leadership role, you will oversee care transitions, discharge planning, and utilization review, ensuring compliance with regulatory and hospital standards. You will mentor staff, analyze outcomes, and partner with physicians and interdisciplinary teams to remove barriers to care. Responsibilities Lead and supervise the case management team, providing coaching and performance feedback. Oversee discharge planning and care transitions to ensure safe, timely patient throughput. Conduct and oversee utilization review to support appropriate level of care and resource use. Collaborate with physicians, nurses, and ancillary staff to coordinate complex care plans. Monitor quality, length of stay, and readmission metrics; implement improvement initiatives. Ensure compliance with regulatory, payer, and hospital case management standards. Serve as a clinical resource for case managers on complex patient cases. Develop and refine case management workflows, policies, and best practices. Educate staff and stakeholders on case management processes and discharge options. Report departmental outcomes and trends to leadership and participate in strategic planning.
