Job Description

LPN:
Clinical Education and Nursing Coordinator The Clinical Education and Nursing Coordinator (LPN) is responsible for coordinating nursing and medical services, providing staff education and competency training, and supporting the health, safety, and quality of care of clients within the rehabilitation program. This position has responsibility for nursing services across two separate program locations and is expected to work between both locations as needed to support consistent clinical practices, staff development, and operational needs. The Coordinator works closely with the Director, Medical Director, and interdisciplinary teams at both locations to ensure effective service delivery, regulatory compliance, and consistent implementation of nursing standards, policies, procedures, and client care practices across the organization. Clinical Care and Medical Coordination Ensure the health, safety, and well-being of all clients. Provide nursing care as needed, including medication administration, nursing assessments, vital sign monitoring, and treatments within scope of practice. Coordinate all medical services, including physician appointments, specialty consultations, diagnostic testing, and follow-up care. Schedule medical appointments, arrange transportation, maintain appointment records, and communicate recommendations to the treatment team. Attend medical appointments as needed to support continuity of care and advocate for client needs. Participate in medical rounds and collaborate with the Medical Director regarding client health concerns. Manage medication systems, including ordering, refills, pharmacy coordination, and oversight of Medication Administration Records (MARs). Monitor implementation of physician orders, treatment plans, and medical protocols. Provide emergency response, first aid, and crisis intervention as needed. Wound and Skin Integrity Management Conduct routine skin assessments and monitor clients at risk for skin breakdown. Provide and oversee wound care treatments as ordered. Ensure repositioning schedules and preventive skin care interventions are implemented and documented. Monitor wound healing progress and promptly report concerns or changes to the Medical Director and leadership team. Maintain accurate wound care documentation and treatment records. Staff Education and Competency Development Serve as the primary clinical educator for Client Coaches and direct care staff. Coordinate and conduct orientation training for new employees. Provide ongoing education related to medication administration, client care, infection control, safety, and vitals. Conduct medication administration training and competency evaluations. Oversee annual training requirements, staff competencies, certifications, and continuing education. Maintain training records and competency documentation. Identify educational needs and provide coaching, mentoring, and corrective training as necessary. Serve as a clinical resource to staff regarding medical care, safety practices, and rehabilitation services. Program Support and Compliance Support implementation of client treatment plans and therapeutic recommendations. Participate in infection control, safety, quality improvement, and risk management activities. Ensure compliance with regulatory, licensing, accreditation, and organizational requirements. Participate in interdisciplinary meetings, staff development activities, and safety committee meetings. Assist with audits, incident reviews, and corrective action planning related to medical and training functions. Professional Responsibilities Maintain professional, ethical, and confidential practices at all times. Communicate effectively with clients, families, staff, physicians, pharmacies, and community providers. Maintain availability for on-call medical consultation as assigned. Demonstrate flexibility to meet the needs of a 24-hour rehabilitation program. Perform other duties as assigned by the Director or organizational leadership.
Qualifications:
Current LPN license in good standing with the State Board of Nursing. Minimum of two years of nursing experience; experience in rehabilitation, behavioral health, or residential settings preferred. Strong organizational, communication, and leadership skills. Proficiency in medical documentation and electronic health record systems. Ability to manage multiple priorities in a fast-paced environment. Current CPR and First Aid certification required.

$54,000.00 - $64,000.00 per year
