Clinical Leadership & Team Support Serve as a resource and escalation point for clinicians in the field Coach and mentor nurses, therapists, and home health aides Support staff through challenging patient situations and transitions of care Conduct case reviews, audits, and field visits Promote a culture of accountability, teamwork, and excellence Quality & Operational Excellence Oversee care plans and regulatory compliance Support quality improvement initiatives and patient outcomes Monitor key metrics including hospitalizations, patient satisfaction, and clinical performance Collaborate with intake, scheduling, and operational teams Participate in strategic initiatives and agency growth Lead Alongside Great People You'll work closely with a team of experienced clinical supervisors who genuinely enjoy supporting one another. Qualifications Required Active Minnesota RN license Previous home health experience Strong knowledge of Medicare regulations, OASIS, and home health compliance Excellent communication and clinical decision-making skills Ability to thrive in a fast-paced, team-oriented environment Preferred Previous leadership, supervisory, charge nurse, or preceptor experience Experience with EMR systems and electronic documentation Passion for coaching and developing clinicians The Best Leaders Multiply Their Impact You can only care for so many patients yourself.
