Monday-Friday, 8-hour shifts (40 hours/week) Job Overview We are seeking an experienced Registered Nurse (RN) Care Manager to join an acute care hospital team. The Care Manager will coordinate patient care, discharge planning, utilization review, and care transitions to ensure high-quality, cost-effective outcomes for patients with complex medical needs. This role works closely with physicians, social workers, utilization management, and interdisciplinary teams to facilitate safe and timely discharges. Responsibilities Perform comprehensive patient assessments and develop individualized care and discharge plans. Coordinate care transitions and discharge planning for hospitalized patients. Participate in multidisciplinary care planning meetings and collaborate with physicians, social workers, and care teams. Identify barriers to discharge and implement appropriate interventions. Coordinate referrals to post-acute care providers and community resources. Educate patients and families on discharge plans, community services, and available resources. Document assessments, care plans, and interventions accurately in Epic EMR. Monitor patient progression, utilization, and discharge readiness while ensuring compliance with hospital policies and regulatory standards. Qualifications Active North Carolina Registered Nurse (RN) License . Minimum 2 years of RN experience . Acute care hospital case management, care management, or discharge planning experience is required. Strong assessment, critical thinking, communication, and care coordination skills. Nursing Diploma, ADN, or ASN required; BSN preferred.
