Babylon + Port Jefferson area. Would need to be present in each specific area when the Manager of Clinical Services is unavailable for that area.

Performs on-site supervisory and teaching visits to evaluate caregivers Provides clinical orientation to caregivers on assigned home care cases. Participates in the training of new nurses and in-services for field staff. Performs initial nursing assessments Counsels patient/caregiver in meeting nursing and related needs. Assigns and coordinates services provided. Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient's condition/needs. Prepares clinical progress notes ensuring any changes in the patient's condition is reported Monitors clinical records for complete and accurate documentation. May include instruction to caregivers and performing skills competency checks.
