Home Health Clinical Director Generation Home Health | Westerville, OH Full-Time | Leadership Position | $90,000-$105,000 + Performance Bonus Generation Home Health is a growing, family-owned home health agency serving Columbus and surrounding communities. We are looking for an experienced Home Health Clinical Director to take ownership of our clinical department and help us build a high-performing, clinically excellent home health organization. This is not a "paperwork manager" position. We are looking for a clinical leader who knows home health, understands OASIS and Medicare, can develop clinicians, recognizes problems before they become deficiencies, and is comfortable holding people accountable. The right person will have significant influence over how our clinical department operates and will work directly with company leadership to build the systems, culture, and standards that will support our continued growth. What You'll DoLead the Clinical Department You will be responsible for the overall clinical performance of Generation Home Health.
You will:
Provide day-to-day clinical leadership to our nursing and therapy teams. Establish clear clinical expectations and standards. Provide clinical guidance for complex patients and challenging situations. Review patient care and identify potential clinical risks. Ensure care is delivered according to physician orders, individualized plans of care, and applicable regulations. Support clinicians when they encounter difficult clinical situations. Develop a culture of accountability, communication, and clinical excellence. Own OASIS & Documentation Quality You will be a key leader in ensuring our clinical documentation is accurate, timely, and defensible.
Responsibilities include:
Overseeing OASIS quality and accuracy. Conducting routine chart audits. Identifying documentation trends and recurring problems. Coaching clinicians on documentation and OASIS. Ensuring Plans of Care accurately reflect patient needs. Monitoring physician orders and required documentation. Addressing documentation problems before they affect patient care, compliance, or reimbursement. Strong OASIS knowledge is required. Manage Clinical Quality & QAPI You will own the clinical quality improvement process.
You will monitor:
Hospitalizations Emergency department utilization Falls Medication-related issues Patient complaints Patient satisfaction Clinical incidents Documentation quality Other applicable clinical outcomes You will identify trends, develop corrective actions, and make sure improvements actually occur. Oversee Admissions & Clinical Operations You will work closely with intake, scheduling, and leadership to make sure appropriate referrals become timely admissions.
You will:
Help evaluate clinical appropriateness of referrals. Assist with complex or high-risk admissions. Ensure appropriate disciplines are ordered. Improve referral-to-admission processes. Monitor Start of Care timeliness. Identify barriers to admission. Help ensure patients receive the appropriate level and frequency of care. Understand PDGM & Utilization You don't need to be a billing expert—but you do need to understand how clinical decisions affect utilization and reimbursement.
You will monitor:

Lead & Develop Our Clinicians You will be responsible for developing our clinical team.
You will:
Participate in hiring clinical staff. Help establish orientation and onboarding. Conduct competency assessments. Provide ongoing coaching. Conduct performance evaluations. Address performance problems. Develop clinical education. Hold clinicians accountable to established standards. Recognize and develop high-performing employees. Help build future clinical leaders within the organization. We want a leader who can coach people up—and have difficult conversations when necessary. Maintain Survey Readiness You will help ensure Generation Home Health is continuously ready for regulatory review.
Responsibilities include:
Monitoring compliance with Medicare and applicable Ohio requirements. Conducting internal clinical audits. Identifying potential deficiencies. Implementing corrective actions. Monitoring compliance with agency policies. Participating in ODH, CMS, accreditation, and payer audits/surveys as applicable. Keeping the clinical department survey-ready year-round. Remain Clinically Engaged Although this is primarily a leadership position, we expect our Clinical Director to remain clinically credible and connected to patient care.
You may occasionally:
Conduct supervisory visits. Complete competency assessments. Assist with complex admissions. Perform clinical audits in the field. Visit patients when senior clinical judgment is needed. Provide temporary clinical coverage when necessary. This position is not intended to carry a full-time patient caseload. Your primary responsibility is leading the clinical department. If you see a problem, we expect you to help solve it—not simply report it. Required Qualifications Current, unrestricted Ohio RN license . Minimum 3 years of professional nursing experience. Minimum 2 years of Medicare-certified home health experience . Previous clinical leadership, supervisory, or management experience. Strong working knowledge of OASIS. Strong understanding of Medicare home health. Working knowledge of PDGM. Strong clinical documentation skills. Excellent communication and organizational skills. Proficiency with electronic medical records. Valid driver's license and reliable transportation. Preferred Qualifications Previous Home Health Clinical Manager, Clinical Director, DON, or similar leadership experience. BSN. COS-C, HCS-O, or similar OASIS certification. Experience with QAPI. Experience with Medicare Advantage. Experience with Medicaid and commercial payers. Experience with Homecare Homebase or another major home health EMR. Previous experience preparing for state, CMS, or accreditation surveys. Demonstrated success developing and retaining clinical staff.
Compensation & Benefits Base Salary:
$90,000-$105,000 annually Exceptional candidates with significant home health leadership experience may be considered above the posted range.
