Job Summary We are seeking a dynamic and compassionate Licensed Practical Nurse (LPN) to step into the vital role of Charge Nurse within our healthcare team. Duties Provide direct patient care, including assessments, vital signs, medication administration, and wound care, ensuring safety and comfort. Supervise nursing staff on your shift, delegating tasks effectively and offering guidance to promote best practices in patient care. Coordinate interdisciplinary communication for comprehensive treatment planning, discharge procedures, and ongoing care needs. Maintain accurate medical documentation using electronic health record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA and long-term care regulations. Oversee clinical procedures including IV infusions, catheterization, sterile processing, and airway management to uphold safety standards. Conduct patient assessments for diverse populations including geriatric patients with dementia or Alzheimer's disease, individuals with developmental disabilities, or those requiring memory or behavioral health care. Lead emergency response efforts utilizing skills in first aid, CPR, acute pain management, trauma protocols (Level I & II trauma experience), and infection control measures. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice. Proven experience working in various healthcare settings such as hospitals (including ICU or emergency medicine), skilled nursing facilities, assisted living communities, or home & community care environments. Demonstrated expertise in critical care scenarios including ICU experience, trauma center procedures, infectious disease care, and acute care management. Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth; proficiency in

coding and medical documentation standards. Strong knowledge of physiology, anatomy, medication administration protocols—including injections—and medical terminology. Experience working with diverse patient populations such as pediatric (PICU), neonatal ventilator management, or adult geriatrics requiring dementia or Alzheimer's care. Excellent caregiver communication skills with the ability to assess patient needs accurately and collaborate effectively with multidisciplinary teams. Additional qualifications preferred include triage experience, hospice or palliative care familiarity, tube feeding management, ventilator support knowledge, and experience working with individuals with disabilities or behavioral health needs.
