•$40.62 an hour 4 hours ago Qualifications Patient management software Technical Proficiency Full Job Description We are seeking a Clinical Care Coordinator (LPN/RN) to join our home health operations team in the Portland Metro Area . In this non-direct patient care role, you will act as a key liaison between patients, physicians, and leadership to coordinate care workflows, manage clinical communication, and support day-to-day administrative operations.

Key Responsibilities Administrative & Workflow Support:
Assist leadership with daily administrative operations and clinic workflow coordination.
Lab Management:
Process, organize, and route incoming lab results to the appropriate clinical team members for timely review.
Clinical Triage:
Conduct phone and administrative triage for patient visit requests to determine urgency and direct care appropriately.
Physician & Patient Communication:
Serve as the primary contact for incoming calls, care inquiries, and follow-ups from healthcare providers and patients.
Care Order Processing:
Track, process, and manage delayed or pending care orders to ensure seamless treatment continuity.
Non-Direct Care Focus:
Execute administrative and care-coordination functions exclusively (no hands-on patient care required).
Requirements & Qualifications Licensure:
Active Licensed Practical Nurse ( LPN ) required (Registered Nurse / RN also considered).
Technical Skills:
Strong computer proficiency and adaptability with clinical software systems.
Professional Attributes:
Highly organized, self-directed, and able to prioritize complex clinical coordination tasks independently.
Preferred Qualifications Industry Experience:
Prior experience in Home Health operations or care management.
Software:
Hands-on experience with HomeCare HomeBase (HCHB) EMR.
