Job Duties:
Facilitate and actively participate in interdisciplinary planning of clinical care of patients and assist with outcome report completion. Serve as care liaison between residents, physicians, care team members, and discharge planner by coordinating, monitoring, and communicating residents' progress. Initiate and implement care plans as necessary through monitoring and re-evaluation to accommodate changes in treatment or progress. Communicate discharge information to physicians and payers with outcome data, as appropriate. Make written and oral reports/recommendations concerning the activities of your shift as required. Ensure that all nursing service personnel comply with their respective job descriptions. Participate in the development, maintenance, and implementation of the facility's quality assurance program for the nursing service department. Participate in facility surveys (inspections) made by authorized government agencies as may be requested. Conduct accident/incident investigations and give completed reports to the Director of Nursing. Perform administrative duties such as completing medical forms, reports, evaluations, studies, and charting. Collaborate with Nurse Leadership Team to provide 24/7 on-call support for nursing team.
