Key Responsibilities:

Evaluate the needs of members, identifying barriers to accessing appropriate care and recommending solutions. Develop ongoing care plans and collaborate with providers to address unmet needs. Coordinate between members, families, and care teams to ensure person-centered care. Provide ongoing follow-up and monitoring of member status and progress towards care goals. Facilitate access to local resources for various services based on assessments. Perform outreach to assess member needs through telephonic, digital, or home visits. Maintain compliance with state, federal, and third-party payer regulations through accurate documentation. Educate members and families on disease processes, care options, and healthcare benefits. Collaborate with leadership to enhance care delivery and quality.

Key Requirements and Technology Experience:

• 4 years of related experience in care management. Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing. RN
• Registered Nurse
• State Licensure and/or Compact State Licensure required.

Experience managing high-risk pregnant women, postpartum mothers, or children. Knowledge of Medicaid/care-management programs.
