About Us We are a collaborative, patient-centered primary care practice providing comprehensive care for individuals and families from birth through geriatrics. Our team is dedicated to promoting wellness, managing chronic conditions, and supporting patients in achieving their best health. Position Summary We are seeking a compassionate and experienced Registered Nurse (RN) / Care Manager to join our office-based primary care team. The ideal candidate will have experience in clinical care coordination, patient education, and managing both acute and chronic health conditions in an outpatient setting. Main Responsibilities Provide patient care and medical triage within the primary care office setting Coordinate care across multiple providers and services Educate patients and families on disease management, preventive care, and treatment plans Administer vaccines and assist with office-based procedures Support patients with behavioral health, substance use, and chronic disease management (such as diabetes) Contribute to a collaborative team environment focused on quality care and patient satisfaction Qualifications Current Registered Nurse (RN) license (required) Experience in a primary care or outpatient setting preferred Strong communication and teaching skills Ability to multitask and work collaboratively within a care team Experience with care management, behavioral health, substance use, or diabetes education a plus Schedule & Benefits Full-time, Monday through Friday — No weekends Supportive, team-oriented work environment Opportunity to make a meaningful difference in the lives of patients and families Join Our Team If you're looking for a rewarding nursing position where you can build lasting relationships and help patients achieve their health goals, we'd love to hear from you!
