Job Description

CareRite Centers is seeking an experienced LPN/RN Supervisor for our Night Shift team. In this leadership role, you will oversee bedside care for post-acute and long-term residents, coordinate and delegate assignments, and support a resident-centered, compassionate care model. Responsibilities include clinical assessments, medication administration, wound care, documentation, and ensuring regulatory compliance. You will mentor staff, promote a dignified and hospitality-driven environment, and collaborate with the interdisciplinary team to drive quality outcomes and continuous improvement. Responsibilities Supervise and support LPNs, RNs, and CNAs during the night shift to ensure safe, high-quality resident care. Perform comprehensive clinical assessments and implement individualized care plans for post-acute and long-term residents. Administer medications and treatments accurately, including IVs and wound care, in accordance with physician orders and facility policy. Ensure timely, accurate documentation in electronic health records and maintain regulatory and compliance standards. Monitor unit workflow, assign and adjust staff duties, and assist with staffing needs and scheduling as needed. Respond to changes in resident condition, coordinate with physicians, and communicate updates to families and the interdisciplinary team. Promote infection prevention, safety, and fall-reduction practices throughout the unit. Provide coaching, mentorship, and real-time clinical guidance to nursing staff to support professional growth and quality outcomes. Participate in quality improvement initiatives and contribute to a culture of continuous improvement in resident experience.
