Oversee medication management including IV infusion, injections, and tube feeding procedures while maintaining strict compliance with HIPAA and confidentiality policies. Conduct comprehensive patient assessments, including physical examinations and diagnostic evaluations such as laboratory specimen collection and processing. Manage electronic health records (PCC systems) documentation accurately using platforms like Epic or Cerner. Ensure compliance with long-term care regulations and safety protocols specific to skilled nursing facilities or assisted living environments. Qualifications Valid LVN license with current state licensure required. Demonstrated expertise working with geriatric patients, individuals with developmental disabilities or memory impairments like Alzheimer's disease. Strong knowledge of medical terminology, physiology, pharmacology related to pain management and infection control practices. Excellent communication skills with the ability to lead a team effectively while maintaining clinical confidentiality policies. Experience working within long-term care regulations or sub-acute care settings; knowledge of hospice or home & community care is a plus.
