The Role We are a newly established, mission-driven agency seeking an experienced, active Virginia-licensed Registered Nurse (RN) to serve as our Assessing RN / Clinical Supervisor. This is a low-stress, highly flexible 1099 independent contractor position designed for a nurse seeking supplemental income outside of grueling hospital shifts.
Important Clinical Scope Note:
GT-Bio-Medical provides non-medical personal care services. There is strictly no skilled nursing care involved—specifically, no wound care and no IV therapy. Your responsibilities focus entirely on client safety evaluations, non-skilled care planning, and caregiver supervision.
Key Responsibilities Initial Intake Assessments:
Conduct in-home, face-to-face evaluations for new clients across the Greater Richmond area to establish baseline safety parameters and non-skilled care needs. Plan of Care (POC)
Development:
Develop and authorize the non-skilled GT-Bio-Medical RN Plan of Care, identifying authorized Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
Task Delegation:
Authorize safe task delegation to qualified field caregivers (PCAs/CNAs).
Supervisory Visits:
Conduct mandatory 90-day supervisory follow-up visits (or 30-day as indicated) to monitor client well-being and evaluate caregiver execution of the Plan of Care.
Escalation Protocols:
Immediately notify GTBM Administration if a client's clinical condition escalates to require skilled nursing interventions, initiating proper transition protocols.
Licensure Oversight:
Serve as the official Assessing RN of record on our Virginia Department of Health (VDH) license and provide baseline availability for clinical consultations. Compensation Structure (1099 Independent Contractor)
Monthly Base Retainer:
$300/month (guarantees official RN standing on our VDH license and baseline clinical availability—must be able to respond to non-emergency clinical administrative questions within 24 hours).
Initial Intake Assessments:
$75 - $100 per client (covers face-to-face home evaluation and Plan of Care authorization).
Supervisory Visits:
$50 - $65 per visit (mandatory 90-day quality, safety, and caregiver evaluations).
Position Requirements Licensure:
Current, active, and unencumbered Registered Nurse (RN) license in the Commonwealth of Virginia.
Experience:
Minimum of 1 to 2 years of clinical nursing experience (home health, case management, or geriatric care preferred).
Background Check:
Must successfully pass a comprehensive background check as required by Virginia state law.
VDH Standing:
Willingness to provide your Virginia RN license number and signature to be listed as the official Assessing RN on our state VDH application and license.
Transportation:
Reliable personal transportation for field visits throughout the Greater Richmond area.
Clinical Judgment:
Strong clinical assessment capabilities, care planning experience, and clear understanding of non-skilled delegation boundaries.
Professional Standards & Presentation Attire & Footwear:
Professional, clean medical scrubs and closed-toe, slip-resistant footwear are mandatory for all in-home assessments, supervisory visits, and field interactions.
Clinical Standards:
Maintain an exemplary standard of personal grooming and infection control suitable for in-home client evaluations.
