The focus of this position will be integrating primary care and chronic disease management (Hepatitis C, Hypertension, Type 2 Diabetes, and Mental Health) within the Opioid Treatment Program (OTP) setting. This position will oversee dispensary operations and staff. This position will also focus on Transitional Care (monitoring patients across different healthcare settings and medical providers).
