This role ensures the delivery of high-quality patient care through effective oversight of clinical operations, regulatory compliance, staff development, quality improvement initiatives, and interdisciplinary collaboration. The Clinical Manager establishes, implements, and evaluates programs and objectives that support organizational goals, promote exceptional patient outcomes, and uphold the mission and values of the organization. Key ResponsibilitiesClinical Operations & Patient Care Oversight Provide leadership and oversight of patient care services, including scheduling, care coordination, referral management, and ongoing assessment of patient needs. Ensure the development, implementation, and timely revision of individualized plans of care. Coordinate and oversee direct and indirect patient care services delivered by clinical staff. Collaborate with intake personnel to review referrals, obtain required documentation, and facilitate timely patient admissions. Support and promote home health services within the healthcare community and referral network. Assist clinical personnel with patient and caregiver assessment, care planning, implementation, and evaluation. Maintain effective communication with attending and referring providers to ensure coordinated, patient-centered care. Review OASIS documentation for accuracy, compliance, and timely completion while providing education and guidance to staff as needed. Address patient concerns and complaints promptly, working toward resolution and escalating issues when appropriate. Provide direct patient care as needed and as directed by Clinical Directors. Quality & Regulatory Compliance Oversee clinical documentation and billing audits to ensure compliance and accuracy. Support Quality Assurance and Performance Improvement (QAPI) initiatives and participate in monitoring quality outcomes. Analyze and present clinical quality data and performance metrics during QAPI meetings. Assist with organizational performance evaluations, productivity monitoring, and quarterly and annual performance reviews. Ensure compliance with all applicable federal, state, and local regulations, licensure requirements, and accreditation standards, including Accreditation Commission for Health Care (ACHC) standards. Promote safe, effective, and high-quality home health services across the organization. Leadership & Staff Development Assist with recruitment, hiring, onboarding, orientation, performance evaluations, coaching, and corrective action processes for clinical and intake team members. Ensure appropriate staffing levels to meet patient care needs and operational demands. Develop, implement, and evaluate orientation programs for new employees. Plan and coordinate ongoing staff education, in-service training, competency validation, and professional development programs. Provide mentorship, guidance, and support to clinical staff to encourage continuous growth and improvement. Conduct competency assessments and on-site visits to evaluate clinical practice and provide real-time coaching and education. Foster a collaborative, professional, and supportive work environment. Strategic & Operational Leadership Collaborate with Clinical Directors and organizational leadership to develop and enhance clinical programs and service offerings. Assist in establishing organizational goals, policies, and procedures. Monitor operational indicators, census trends, and patient volume changes to identify staffing, revenue, and expense implications. Manage clinical supply processes, including inventory oversight, ordering, and cost management. Lead and participate in interdisciplinary team meetings, departmental meetings, staff meetings, and quality improvement initiatives. Collaborate effectively with all members of the clinical and leadership teams. Administrative Responsibilities Maintain current professional licensure, certifications, and credentials. Participate in on-call responsibilities, including evenings, weekends, and holidays as assigned. Travel to satellite locations as needed. Perform other duties as assigned. Qualifications Education & Licensure Current Wisconsin licensure as one of the following: Registered Nurse (RN) Physical Therapist (PT) Occupational Therapist (OT) Speech-Language Pathologist (SLP) Nursing candidates must hold a degree from an accredited nursing program. Therapy candidates must possess, at minimum, a bachelor's degree from an accredited educational program in their respective discipline. Current American Heart Association (AHA) Basic Life Support (BLS) certification. Experience Minimum of two (2) years of experience in a home health setting. Minimum of one (1) year of leadership or supervisory experience in a healthcare environment. Knowledge, Skills & Abilities Thorough understanding of home health regulations, reimbursement, compliance requirements, and accreditation standards. Strong leadership, supervisory, and team development skills. Excellent organizational, problem-solving, and time management abilities. Ability to prioritize multiple responsibilities in a fast-paced environment. Strong verbal and written communication skills. Ability to build and maintain collaborative relationships with staff, providers, patients, families, and community partners. Demonstrated attention to detail and commitment to quality outcomes. Ability to market services and represent the organization professionally within the community. Additional Requirements Valid driver's license with reliable transportation. Automobile insurance that meets organizational and state requirements. Willingness and ability to travel to satellite locations, including locations more than two hours away, as needed. Physical Demands & Work Environment Mobility Requirements Ability to walk and stand for extended periods. Ability to kneel, squat, stoop, bend, and change positions regularly throughout the workday.
