Job Overview We are seeking a dynamic and compassionate Home Health Clinical Manager - Registered Nurse to lead and coordinate high-quality patient care within our home health services. This vital role combines clinical expertise with leadership skills to oversee nursing staff, ensure compliance with healthcare regulations, and promote optimal patient outcomes. Responsibilities Lead and supervise a team of registered nurses and home health aides, ensuring effective care delivery aligned with organizational policies and healthcare regulations. Develop, implement, and evaluate individualized care plans for diverse patient populations, including those with complex needs such as dementia, geriatrics, behavioral health issues, or developmental disabilities. Oversee patient assessments, ensuring thorough physical examinations, vital signs monitoring, medication administration, IV infusion management, catheter care, tube feeding, and ventilator support as needed. Ensure compliance with HIPAA privacy standards, state healthcare regulations, and nursing policy requirements across all documentation and clinical practices. Coordinate interdisciplinary collaboration for discharge planning, case management, utilization review, and utilization management to optimize resource use and patient outcomes. Manage staff scheduling, ensure training compliance, supervising nurse management activities to foster professional growth and team cohesion. This position will still require some patient care. Qualifications Valid Registered Nurse (RN) license in good standing with current state registration. Proven leadership experience managing nursing teams within home health or related settings; supervisory experience preferred. Clinical background including previous home health experience, hospital medicine, emergency medicine or medical-surgical units. Demonstrated expertise in patient assessment techniques such as vital signs monitoring, physical examinations, diagnostic evaluation, and physiology knowledge. Proficiency with EMR/EHR systems like Epic or Cerner; familiarity with
