Key Responsibilities Comprehensive Evaluations:
Assess patients' physical, cognitive, and emotional needs to design highly tailored, goal-oriented treatment plans.
ADL & Functional Independence:
Assist and train patients in activities of daily living (ADLs)—including dressing, grooming, and mobility—to maximize autonomy.
Multi-Setting Care Delivery:
Seamlessly implement therapeutic interventions across inpatient units, outpatient clinics, school-based environments, home health, and skilled nursing facilities (SNFs).
Pediatric & Early Intervention:
Support early intervention programs for children exhibiting developmental delays or autism spectrum disorders.
Continuum of Care & Discharge:
Collaborate on case management and discharge planning by evaluating patient progress, recommending home modifications, and coordinating post-acute referrals.
Interdisciplinary Collaboration:
Work alongside speech therapists, physiotherapists, and medical staff to provide integrated, holistic patient care.
Documentation & Compliance:
Maintain meticulous, timely documentation of assessments, treatment milestones, and care plans in strict compliance with healthcare regulations.
Education & Advocacy:
Educate patients, families, and caregivers on adaptive techniques, therapeutic exercises, and home safety measures. ##
Job Requirements & Skills Clinical Knowledge:
Strong foundational knowledge of anatomy, physiology, medical terminology, and advanced functional assessment techniques.
Setting Adaptability:
Proven competence handling complex cases across various settings (e.g., ICU, acute care, outpatient, schools, or home health).
Diverse Populations:
Experience working with a broad patient demographic, including pediatrics, geriatrics, individuals with disabilities, and neurodivergent populations (e.g., autism spectrum disorders).
Case Management:
Familiarity with discharge planning processes, quality improvement initiatives, and healthcare compliance.
Integrated Care:
Knowledge of speech therapy and physiotherapy principles is highly advantageous for our collaborative care model.
