Active In-Home Therapy is a leading provider of home health services committed to delivering compassionate and high-quality care to individuals in the comfort of their homes. We are currently seeking a skilled and dedicated Home Health Occupational Therapist to join our dynamic team.
Position Overview:
As a Home Health Occupational Therapist, you will play a crucial role in providing specialized occupational therapy services to our patients within the familiar and supportive environment of their homes. You will work closely with patients to assess their functional abilities, develop personalized treatment plans, and implement therapeutic activities to promote independence and improve overall quality of life. This is a PRN position, meaning you will be working with us on an as-needed basis. You will be hired as a W2 employee, not as an independent contractor. We offer flexible scheduling, and you can freely choose the areas you prefer to cover. We would make an excellent addition to your full-time job!
Key Responsibilities:
Conduct thorough assessments of patients' functional abilities to determine rehabilitation needs. Develop and implement individualized occupational therapy treatment plans with the healthcare team. Provide hands-on occupational therapy interventions to address activities of daily living, fine motor skills, and adaptive techniques. Educate patients and their families on adaptive strategies, assistive devices, and techniques to enhance independence in daily activities. Monitor and document patients' progress, adjusting treatment plans as necessary. Collaborate with other healthcare professionals, including physicians, nurses, and physical therapists, to ensure comprehensive patient care. Maintain accurate and up-to-date patient records. Communicate effectively with patients, families, and caregivers to ensure a positive and supportive therapeutic environment.
Qualifications:
Master's degree in Occupational Therapy from an accredited institution. Current state licensure as an Occupational Therapist. Prior experience in home health or a related setting is preferred, but not required. Strong clinical and critical thinking skills. Excellent communication and interpersonal abilities. Ability to work independently and as part of a collaborative healthcare team.
