Job Description

Job Overview We are seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will have a strong background in patient care and a passion for helping individuals achieve their maximum potential through therapeutic interventions. This role involves working with diverse populations geriatrics, and individuals with disabilities, in home health environments. Responsibilities Assess patients' needs and develop individualized treatment plans to enhance their ability to perform daily activities (ADLs). Implement therapeutic interventions aimed at improving patients' physical, emotional, and cognitive functioning. Collaborate with multidisciplinary teams to ensure comprehensive patient care and effective discharge planning. Provide education and support to patients and their families regarding care plans and rehabilitation goals. Maintain accurate documentation of patient progress and treatment outcomes in accordance with regulatory standards. Stay current with best practices in occupational therapy, including techniques for working with individuals with autism, acute care settings, and early intervention strategies. Participate in case management activities to coordinate care for patients across various healthcare settings. Skills Proficiency in assisting with activities of daily living (ADLs) and developing care plans tailored to individual needs. Knowledge of medical terminology, anatomy, physiology, and the principles of occupational health. Experience working with diverse populations including geriatrics,individuals with disabilities, and those requiring post-acute care. Familiarity with various therapeutic settings such as inpatient units, skilled nursing facilities, and home health environments. Ability to provide speech therapy support when necessary and collaborate effectively within a multidisciplinary team. Strong communication skills to effectively educate patients and families about treatment processes and goals. Experience in case management is preferred along with a commitment to ongoing professional development in the field of occupational therapy. From $70.00 per hour
