Home Health Occupational Therapist (OT) - Per Visit / 1099 Contractor Leona Rehab Group, LLC is a therapist-focused healthcare staffing company serving home health agencies throughout the Inland Empire and surrounding areas. Therapist-Led Support As a company founded by a home health clinician, we understand the challenges of documentation, scheduling, communication, and patient care in the field. Streamlined Communication Our team works to ensure referrals, patient information, and scheduling details are communicated clearly and efficiently. Independence with Support Enjoy the flexibility of contract work while having a responsive team available when questions arise. Responsibilities Perform physical therapy evaluations and reassessments in patients' homes Develop individualized plans of care Collaborate with physicians, nurses, therapists, caregivers, and agency staff Complete documentation accurately and in a timely manner Educate patients and caregivers regarding treatment plans and safety recommendations Maintain compliance with Medicare, state, and agency regulations Qualifications Current California Physical Therapist (PT) license in good standing Current CPR certification Valid driver's license and reliable transportation Strong communication and time-management skills Home health experience preferred but not required New graduates are encouraged to apply What We're Looking For We are seeking therapists who are: Compassionate and patient-centered Reliable and responsive Organized and self-motivated Professional and collaborative Comfortable with technology and electronic documentation Committed to delivering exceptional care Job Type Per Diem Part-Time Contract (1099) Benefits Flexible schedule Referral bonus opportunities Professional growth opportunities Competitive per-visit compensation At Leona Rehab Group , we believe great patient care starts with supporting great clinicians.
