Overview Partners CA Home Health is seeking a qualified and compassionate Occupational Therapist (OT) - Per Diem to provide skilled occupational therapy services to patients in their homes. The Occupational Therapist will evaluate patients' ability to safely complete daily activities, develop individualized treatment plans, provide therapeutic interventions, and help patients improve independence, safety, and overall quality of life. This position is ideal for an Occupational Therapist who is comfortable working independently in the home health setting while collaborating with nurses, physical therapists, speech therapists, physicians, and other members of the interdisciplinary care team. Duties Perform comprehensive occupational therapy evaluations in the patient's home. Assess the patient's ability to complete activities of daily living such as dressing, bathing, grooming, toileting, meal preparation, and household tasks. Evaluate upper-extremity function, coordination, strength, endurance, cognition, safety awareness, and functional mobility as appropriate. Develop individualized occupational therapy treatment plans based on physician orders, patient needs, and established goals. Provide skilled therapeutic interventions designed to improve independence with daily activities. Train patients in adaptive techniques and the use of assistive or adaptive equipment when appropriate. Provide education on home safety, fall prevention, energy conservation, body mechanics, and techniques for completing daily activities safely. Educate patients and caregivers on exercises, home programs, safety precautions, and strategies to support continued progress. Monitor patient progress and modify treatment plans as needed. Perform reassessments and discharge evaluations when appropriate. Communicate significant changes in patient condition to the physician and other members of the care team. Assist with discharge planning and coordination of care. Collaborate with physical therapists, speech therapists, nurses, home health aides, physicians, and other healthcare professionals. Complete all clinical documentation accurately and within agency-required timeframes. Maintain compliance with Partners CA Home Health policies, payer requirements, and applicable professional standards. Requirements Current and unrestricted California Occupational Therapist license . Graduate of an accredited Occupational Therapy program. Home health experience preferred. Experience working with geriatric, orthopedic, neurological, and medically complex patients is preferred. Strong knowledge of activities of daily living, functional assessment, adaptive techniques, and home safety. Strong clinical assessment and treatment-planning skills. Ability to work independently and manage a flexible patient schedule. Comfortable using electronic medical record systems. Strong communication and patient education skills. Reliable transportation and ability to travel to patient homes within the assigned service area. Current CPR/BLS certification, if required by agency policy. Must meet all Partners CA Home Health employment and credentialing requirements. Schedule Per Diem / As Needed Patient visits are assigned based on agency census, geographic location, patient needs, and therapist availability. No minimum number of visits is guaranteed. Compensation Per-visit compensation based on the current Partners CA Home Health per diem pay schedule.
