South Pacific Rehab Services is seeking a compassionate Occupational Therapist to provide resident-centered care in skilled nursing and post-acute facilities. In this on-site role, you will evaluate residents, develop individualized treatment plans, and deliver one-on-one interventions that promote functional independence, safety, and quality of life. You'll collaborate closely with an interdisciplinary team, document in EMR, and educate residents and caregivers on adaptive strategies. Responsibilities Evaluate residents in skilled nursing and post-acute settings and develop individualized treatment plans focused on functional goals Deliver one-on-one occupational therapy interventions to improve ADLs, mobility, cognition, and safety Document assessments, progress notes, and care plans accurately and timely in EMRCollaborate with nursing, rehab, and medical teams to support coordinated, patient-first care Educate residents, families, and caregivers on adaptive strategies, equipment, and safety Contribute to discharge planning and recommendations for continued services or equipment needs Participate in clinical education, mentoring, and quality improvement initiatives Required Skills Occupational therapy evaluation and treatment Care planning in SNF/post-acute settings ADL and functional mobility training Cognitive and perceptual retraining Therapeutic exercise and neuromuscular re-education Wheelchair positioning and adaptive equipment Documentation in EMR systems Interdisciplinary team collaboration Fall prevention and safety training Family and caregiver education
