Job Summary Join our dynamic team as a Home Health Occupational Therapist in Sacramento County, where you'll have the opportunity to make a meaningful impact on the lives of individuals with physical, developmental, and cognitive challenges. As an integral member of our healthcare team, you will provide comprehensive therapy services, including assessment, treatment planning, and patient education. Responsibilities:
Conduct comprehensive assessments of patients' physical, emotional, and environmental needs. Develop and implement individualized therapy plans to address specific conditions and enhance daily living skills. Provide direct therapy services to clients in individual and group settings. Educate patients and their families on strategies to improve functional independence. Monitor patients' progress and adjust therapy plans as needed to promote optimal outcomes. Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and holistic care. Document patient care accurately and maintain confidentiality of patient information. Provide emotional support and counseling to patients and their families to promote well-being and coping strategies. Participate in interdisciplinary team meetings and contribute to the development of quality improvement initiatives.
Occupational Therapist Qualifications:
1-2 years Direct Home Health experience Current Occupational Therapist license in the state of CA. 1-2 years of clinical experience in occupational therapy, preferably in a similar setting. NBCOT certification preferred. BLS/CPR certification required. Valid CA Driver's license
