Occupational Therapist (OT) - Home Health SummitWest Care | Mesa County, CO Full-Time | Flexible Daytime Schedule $48-55/hour DOE + Mileage Reimbursement Help Patients Maintain Independence Where It Matters Most. SummitWest Care is a local nonprofit home health agency seeking a compassionate Occupational Therapist (OT) to provide one-on-one care throughout Mesa County. Home health offers a different therapy experience. Instead of focusing on productivity quotas and packed schedules, you'll have the opportunity to focus on what matters most: helping patients safely and confidently navigate everyday life in their own homes. You'll build meaningful relationships, see the results of your work firsthand, and develop treatment plans tailored to each patient's real-world environment and goals. Responsibilities:
Perform occupational therapy evaluations, reassessments, recertifications, and discharges Develop and implement individualized treatment plans focused on safety, function, and independence Assess patients' ability to performs ADLs Conduct home safety evaluations and recommend adaptive equipment or environmental modifications Educate patients and caregivers on home programs, mobility strategies, and safety techniques Monitor patient progress and adjust treatment plans as appropriate Collaborate with nurses, therapists, caregivers, physicians, and family Supervise and support COTAs when applicable Complete accurate and timely clinical documentation
Qualifications:
Current Colorado Occupational Therapist (OT) license in good standing Graduate of an accredited Occupational Therapy program Strong clinical assessment, communication, and documentation skills Ability to work independently in a home health environment Excellent time-management and organizational skills Valid driver's license, reliable transportation, and auto insurance Home health experience is a bonus, but not required!
