3005100492-2 Apply for this job now Job Description Powerback Rehabilitation is seeking an Occupational Therapist to deliver patient-centered, outcome-driven care across post-acute, outpatient, and home-based settings. In this role, you will evaluate and treat patients to restore function, maximize independence, and improve quality of life through individualized, evidence-based interventions. You'll work in a collaborative, clinically driven environment alongside PT, SLP, nursing, and medical teams, contributing to measurable outcomes and continuous improvement. Responsibilities Provide evidence-based occupational therapy evaluations and individualized treatment plans for post-acute, outpatient, and home-based patients Deliver one-on-one and group interventions to improve function, independence, and safety in daily activities Collaborate with physical therapy, speech therapy, nursing, and medical teams on coordinated care plans Document assessments, goals, progress notes, and discharge plans in accordance with regulatory and payer requirements Educate patients, families, and caregivers on home programs, adaptive equipment, and compensatory strategies Participate in interdisciplinary case conferences and contribute to measurable outcome tracking Supervise and mentor therapy assistants or support staff as assigned Maintain compliance with clinical best practices, ethical standards, and organizational policies Required Skills Occupational therapy evaluation and treatment planning ADL and IADL retraining Neuromuscular re-education Orthopedic and post-surgical rehabilitation Functional cognition and vision rehabilitation Use of adaptive equipment and assistive technology Electronic medical record (EMR) documentation Interdisciplinary care coordination Home safety assessment and discharge planning Patient and caregiver education
