Skilled Nursing Facility Location:
Lake Worth, FL We're seeking an experienced Occupational Therapist (OT) to join a reputable Skilled Nursing Facility in Lake Worth, FL. This is a full-time, permanent role for an OT who is comfortable working in a SNF / long-term care environment and treating a geriatric and post-acute population. Position Overview The Occupational Therapist will evaluate residents and provide skilled therapy services focused on improving independence with activities of daily living, functional mobility, and safety. Key Responsibilities Conduct occupational therapy evaluations and develop individualized treatment plans Provide skilled OT services to short-term rehab and long-term care residents Address ADLs, functional mobility, cognition, and adaptive equipment needs Collaborate with PTs, SLPs, nursing staff, and physicians as part of the care team Document evaluations, treatments, and progress in compliance with facility and regulatory standards Participate in care planning and discharge planning as appropriate Qualifications Active Florida Occupational Therapist license Prior experience in a Skilled Nursing Facility or Long-Term Care setting required Strong documentation and time-management skills Ability to work independently in a fast-paced clinical environment Experience with geriatric and post-acute rehab patients preferred What's Offered Competitive compensation based on experience Stable, full-time permanent position (not a contract role) Supportive rehab team and consistent caseload Opportunity to work in a well-established SNF with strong leadership This is a great opportunity for an OT who enjoys SNF-based rehab and is looking for long-term stability in the Lake Worth area.
