• Assists in the development and implementation of the interdisciplinary client care plan, as it pertains to occupational therapy, in consultation with the physician.
• Provides skilled occupational therapy services as outlined and in accordance with the physician's orders.
• Evaluates the client's level of function using diagnostic and prognostic procedures.
• Guides and instructs clients in prescribed therapeutic self-care and creative activities to promote independence and improve physical and mental functioning.
• Instructs clients in the care and use of wheelchairs, braces, splints, prosthetic devices, and orthotic devices.
• Teaches, supervises, and counsels clients and their families regarding the occupational therapy program and related home care needs.
• Establishes treatment goals based on the client's current level of function and potential for improvement, and performs re-evaluations as indicated.
• Coordinates and arranges for outpatient services, as appropriate.
• Evaluates the effectiveness of occupational therapy interventions and updates the client care plan as needed.
• Communicates regularly with the physician regarding the client's status and obtains additional orders when necessary.
• Prepares clinical and progress notes on the day of each visit.
• Communicates with other team members and provides instruction in occupational therapy techniques, when appropriate.
• Participate in agency activities, in-service education, and meetings, as appropriate.
• Supervises the Occupational Therapy Assistant at least once per month.
• Collaborates with staff, the client, and the physician in discharge planning and completes the discharge summary.
• Performs all other duties as assigned.
